Best Practice Document -
A First Steps document that assists providers in understanding their
roles and responsibilities within the program, as well as helping
families make informed decisions about their child’s services.

Provider Missing
Authorization Request - Connect directly with the SPOE office to
address problems with missing authorizations.
Provider Update
Form - This form is to be used if there is a change to be made to
provider enrollment records, and should be sent to EDS once completed.
Pat On the Back
Form- Nominate a fellow colleague with a Pat on the Back,
for the wonderful job that they do. They will be recognized by the
LPCC in Cluster G, and be eligible for an award at the Annual Meeting in
the Spring.

IDEA
- The
Individuals with Disabilities Act is a law ensuring services to children
with disabilities throughout the nation. IDEA governs how states
provide early intervention and special education to eligible infants,
toddlers, children and youth with disabilities. Infants and toddlers
with disabilities (birth-36 months) and their families receive services
under IDEA Part C. Children and youth (ages 3-21) receive special
education and related services under IDEA Part B.
FERPA-
The Family Educational Rights and Privacy Act is a federal law that
gives parents the right to have access to their children’s education
records, the right to have records amended, and the right to have
control over the disclosure of personally identifiable information from
the education records.
